Under-eye filler is a hyaluronic acid injectable treatment used to improve tear trough hollowing and loss of support beneath the eyes.
The tear trough is the natural depression that runs from the inner corner of the eye into the upper cheek. When this area becomes deeper due to genetics, ageing, volume loss or changes in the surrounding facial tissues, it can create a persistent shadow that makes the eyes appear tired even when you are well rested.
Carefully placed filler can restore subtle support beneath the hollow, soften the transition between the lower eyelid and cheek, and reduce shadowing without changing the natural character of the eyes.
Importantly, under-eye filler is not suitable for every type of dark circle or under-eye concern. Pigmentation, prominent eye bags, fluid retention or significant skin laxity may require a different treatment approach.

How does it work?
Restores volume within the tear trough
The tear trough is a natural depression extending from the inner corner of the eye toward the upper cheek. It can become more pronounced through congenital anatomy, loss of soft-tissue volume, mid-face descent and reduced bony support with age. Hyaluronic acid filler is used to restore carefully controlled volume within or around this hollow.
Improves the lid–cheek transition
A youthful lower eyelid usually blends smoothly into the upper cheek. When a tear trough develops, this transition becomes more abrupt and creates a visible groove. Filler can soften that junction, making the under-eye contour appear smoother and less sunken.
Reduces shadowing rather than pigmentation
Much of the “darkness” associated with tear troughs is caused by shadow from the depression itself. By reducing the depth of the hollow, filler can improve this shadowing and create a more rested appearance. It will not, however, remove true skin pigmentation or vascular dark circles.
May require mid-face support first
Under-eye hollowing does not always originate solely beneath the eye. Loss of cheek volume and support can expose the tear trough further. In these cases, supporting the mid-face may improve the under-eye area and reduce the amount of filler required directly within the tear trough.
Requires conservative product placement
The under-eye has thin skin, very little subcutaneous tissue and a tendency to retain fluid. For this reason, small amounts of an appropriate hyaluronic acid filler are generally used, with overcorrection avoided. More hydrophilic or excessively superficial filler can contribute to puffiness or visible irregularity.
Creates a smoother, more rested appearance
The aim is not to create volume around the eyes. It is to restore a smoother anatomical transition between the lower eyelid and cheek, reducing hollowing and associated shadow while preserving the natural shape of the eye.

Is under-eye filler suitable for dark circles? Sometimes. If the darkness is mainly caused by tear trough hollowing and shadowing, filler may improve the appearance by smoothing the transition between the lower eyelid and cheek. If the darkness is primarily due to pigmentation or visible blood vessels, filler may have limited benefit.
Can under-eye filler treat eye bags? Not always. Prominent eye bags are usually caused by protruding fat pads, fluid retention or skin laxity. Adding filler directly beneath an existing bag can sometimes make the area appear heavier, which is why the underlying anatomy needs to be assessed carefully.
Who is a good candidate for tear trough filler? It is generally most suitable for clients with a clear under-eye hollow, good skin quality and relatively little puffiness. Clients with significant fluid retention, large eye bags or marked skin laxity may be better suited to another treatment approach.
Will I need cheek filler as well? Not necessarily. However, some under-eye hollowing is partly caused by loss of support in the mid-face. In these cases, restoring cheek support first can improve the tear trough more naturally and may reduce the amount of filler needed directly under the eye.
How long does under-eye filler last? Results commonly last around 12-16 months, although longevity varies depending on the product used, metabolism, anatomy and individual tissue response.
Is there downtime after under-eye filler? Mild swelling, tenderness or bruising can occur. The under-eye area can also hold fluid more easily than other parts of the face, so temporary puffiness may be more noticeable in some clients.
